Long COVID Disability Recognition Gap: Millions Economically Excluded While Science Establishes the Condition
Problem Definition
Millions of Long COVID patients are caught between a scientifically established condition and institutional systems (insurance, disability, employment law) that have not adapted to recognise it.
The result is economic devastation: patients too ill to work but unable to access disability support, insurance coverage denied for treatments that lack an approved diagnostic code, and employers terminating employees who cannot document their condition with a test result.
Long COVID has no definitive diagnostic biomarker — it is a clinical diagnosis based on symptom persistence. This is not scientifically unusual (fibromyalgia, ME/CFS follow the same pattern) but it creates systematic institutional exclusion.
Root Causes
Absence of a validated biomarker means insurance systems default to denying claims as "unverifiable"
Disability determination processes require documented functional impairment on standardised instruments not validated for Long COVID
ICD coding for Long COVID (U09.9) exists but is rarely used by physicians unfamiliar with the condition
Employer sick leave policies have maximum durations (typically 12-26 weeks) that do not accommodate variable Long COVID recovery timelines
