Task-Shifted Mental Health: Train 1 Million Community Health Workers to Deliver Evidence-Based Care
Description
5 million more psychiatrists to be trained (30+ year horizon). It is to systematize task-shifting: training 1 million community health workers (many already embedded in rural clinics, schools, and primary care) to deliver evidence-based mental health interventions under remote psychiatrist supervision.
The framework: (1) Standardized training modules in cognitive behavioral therapy (CBT), problem-solving therapy, and psychoeducation — evidence-based, non-specialist-deliverable interventions targeting the 80% of mental illness that is mild-to-moderate depression and anxiety.
(2) Digital supervision platform: Community health workers use simple smartphones to document cases, upload session notes, and receive asynchronous psychiatrist feedback. Reduces specialist time required per case from 60 minutes (traditional therapy) to 15 minutes (supervision).
(3) Task-shifted service delivery in schools (school health workers delivering mental health screening and counseling to students), primary care clinics (health workers trained to diagnose and treat depression and anxiety alongside physical health services), and community-based settings (peer-support groups facilitated by trained community members).
(4) Licensing and certification: New credentials for 'Mental Health Technician' and 'Community Health Psychiatrist' recognized by national health systems and compensated via health insurance.
Implementation Pathway
Phase 1 (2025-2027)
- • Train 100K community health workers across 10 pilot countries
- • Develop digital supervision platform
- • Establish licensing framework
Phase 2 (2027-2032)
- • Scale to 500K trained workers globally
- • Integrate into national health system reimbursement in 30 countries
- • Achieve 40% coverage of mild-to-moderate depression/anxiety in pilot countries
Phase 3 (2032+)
- • 1M+ community health workers delivering mental health care
- • Task-shifted care becomes standard model in low/middle-income countries
