Post-Viral Precision Medicine Initiative: Research Cohorts, Biomarker Discovery, and Decentralized Treatment Networks
Description
Establish a global Post-Viral Precision Medicine Initiative (PPMI) combining three tightly integrated components: (1) Unified International Long COVID Cohorts: Fund and harmonize 20 existing Long COVID cohorts across 15 countries into a single 100,000-person international research cohort with standardized phenotyping (symptom batteries, functional capacity, biomarker collection, and serial follow-up).
Use federated data governance to preserve patient privacy while enabling cross-national analysis. (2) Biomarker Discovery Engine: Fund 50 biomarker discovery projects across the 6 leading mechanistic hypotheses (viral persistence, autoimmunity, microclots, dysbiosis, mitochondrial dysfunction, neuroinflammation) using the cohort data and tissue samples.
Structure as a 'winner-take-all' competition where leading mechanistic hypotheses receive scaled-up funding for validation and therapeutic development.
(3) Decentralized Treatment Networks: Establish Post-Viral Care Centers in 100 major cities that provide integrated diagnostic protocols (multi-system biomarker testing, functional imaging, immunophenotyping) and mechanism-based experimental therapies (anticoagulation for microclot hypothesis, immunotherapy trials for autoimmunity hypothesis, probiotics + dietary interventions for dysbiosis).
Networked telemedicine enables access from underserved rural areas.
Implementation Pathway
Phase 1 (2026–2027)
- • International cohort harmonization complete
- • 20 biomarker discovery projects launched
- • 10 Post-Viral Care Centers pilot in major cities
Phase 2 (2027–2030)
- • 100,000-person cohort fully enrolled
- • 5 leading mechanistic hypotheses advanced to Phase 2 clinical trials
- • 50 treatment centers operational globally
Phase 3 (2030+)
- • Precision medicine stratification based on mechanistic phenotypes
- • Standard-of-care treatments emerging from clinical trials
